SEND US A PICTURE (jpg) OF THE PLUG WITH THE CODE NUMBER
CLEARLY SHOWING AND A PICTURE OF THE RAVELCO ITSELF, MOUNTED IN
YOUR VEHICLE.

FORM WITHOUT A SALES RECEIPT OR A DETAILED EXPLANATION
