1.) Send $75.00 per plug plus shipping (US Funds only). YOUR CHECK MUST BE DRAWN ON A US BANK, WE CANNOT ACCEPT ANY CHECKS THAT ARE NOT IN US FUNDS AND DRAWN ON A US BANK. (Some Countries use checks that state "US FUNDS" but we cannot accept them if they are not drawn on an actual US BANK.)

3.) Ravelco Plug Order Form - If you do not have or cannot find your original plug order card, please print out the yellow Ravelco Plug Order Form on the bottom of this page and send it in completed with:

a.) Your name.
b.) Your physical address (we cannot ship to a PO Box).
c.) Quantity of extra plugs desired in the space provided.
d.) Your Code Number in the box provided on the Ravelco Plug Order Form.

4.) CODE NUMBER.  (Print code number in box on yellow card) *CODE NUMBER - It is your responsibility to make sure the code number in your plug matches the code number written on your yellow order card.

You may find the code number in either of the following two places.
   a.) The code number is stamped on the removable male plug located (between the inside pins) on the plug itself. 
   b.) The code number is also stamped on the face of the female receptacle located (between the rivets) that is
        mounted in the vehicle. 
   
5.) Copy of your driver's license.

6.) Phone Number (In case we have to get in contact with you).

7.) Email address. (we do not use your email address for anything other than UPS notifying you)

PLEASE SEND ORDER BY REGULAR U.S. POSTAL SERVICE MAIL ONLY. Please do not send U.S. REGISTERED OR CERTIFIED MAIL or any other delivery service that has to be signed for. If you send an order that has to be signed for, it will delay your order immensely.  Plus, our local United States Post Office here in Richmond, Texas will lose it.
